THE CLIENT INTAKE FORM AND CONTRAINDICATION FORM MUST BE PRINTED OUT, COMPLETED AND BROUGHT TO YOUR INITIAL VISIT. The Colon Hydrotherapy:Q&A’s can be printed out and kept for your reference. It is a good idea to review the material BEFORE and AFTER your treatment.
